Offer In Compromise

Individual taxpayers or business entities should submit form OIC-100 Web-Fill "Offer In Compromise" as part of the Offer In Compromise packet. All sections of the form must be completed, and it should include appropriate identification numbers, signature, and detailed explanation of circumstances. Items that do not apply should be notated with "N/A." If the source of funds is "third party," please attach form OIC-102 Third Party Affirmation to the OIC-100.
