Individual taxpayers must submit the Collection Information Statement for Individuals (Form RO-1062), with sections 1 through 10 completed. If a taxpayer is Self-Employed sections 1 through 12 must be completed. Form RO-1062 is used to obtain current financial information necessary for determining how a wage earner or self-employed individual can satisfy an outstanding tax liability.
